Media Agreement

Please check the following in agreement with the terms of the SSVEC Foundation Educational Entity Grant:

I agree to credit SSVEC in all publicity, advertising, and public notification about this project with the following credit line and SSVEC logo (which will be provided): “Funded by an SSVEC Foundation grant.”

I agree to promote this grant on social media platforms (i.e., Twitter, Facebook, Instagram) to tag #SSVECEEG, SSVEC, your school, and school PTO. Media posts and photos may be used in SSVEC publicity.

I agree to make sure that any student represented in photos or media is allowed to be photographed and have their photograph shared on public platforms. An Image Authorization and Release Form must be signed for any photo/video subjects included in any media provided to SSVEC.

I agree to provide photo and/or video documentation of this project, in the manner requested by SSVEC (i.e., Google upload) in the highest quality possible, of my project in action to SSVEC. Media posts and photos may be used in SSVEC publicity.

I agree to share the receipt of this grant, along with outcomes, with parents of participating students or other pertinent parties.

I agree to provide a testimonial statement as requested. Testimonials may be used in SSVEC materials.

I agree to provide a final report (template will be provided) on the outcomes of this project to SSVEC within 30 days of project completion.

I agree to allow SSVEC to highlight my program's accomplishments in print and other media.

Final Acknowledgement

“I hereby certify that all data and representations made in this application and its attachments are true and correct to the best of my knowledge and ability. Any grant funds received in connection with this application will be expended as described. In the event any modification needs to be made, I will seek guidance from the Sulphur Springs Valley Electric Cooperative Foundation. All funding received from this grant will be utilized within the timeframe indicated on this application. Any unused funds must be returned to the SSVEC Foundation".
